A Case of Darier¡¯s Disease Misdiagnosed as Psoriasis

Abstract
Darier¡¯s disease manifests as erythematous to brownish keratotic papules and plaques predominantly on the seborrheic area and distal extremities. Thickened plaques in Darier¡¯s disease might be misdiagnosed as psoriasis, especially if characteristic histopathologic findings are focal and overlooked. A 62-year-old woman presented with painful erythematous papules, crusted plaques and, erosions on the face, trunk, and extremities. Symptoms started in her adolescence. She was diagnosed with psoriasis through skin biopsy at other clinic 15 years ago and treated with acitretin but, her symptoms were wax and wane. Histopathologic examinations in our department demonstrated acantholytic dyskeratosis with corps ronds and grains. These findings were compatible with Darier¡¯s disease. She was treated with oral isotretinoin 20 mg for 4 months, which induced partial improvement. After that, she has been treated with acitretin 20 mg with further improvement. Here we report a case of Darier¡¯s disease, which is sometimes misdiagnosed as psoriasis because both disorders are responsive to acitretin.
